I think that plugin was meant for Leadwerks 2.X. Since version 3.0 I think the .mdl format has changed a bit. Try exporting to .FBX and see if it works from there. I could be wrong though.

Using Leadwerks Professional Edition (Beta), mainly using C++.

Windows 10 / Linux Mint, Visual Studio 2017. GPU: NVidia GeForce GTX970, CPU: Intel i7 7700K @ 4.20 GHz

I try exporting to .FBX, no result.

 

i think you must uv unwrap in blender
